Day of the Dead - head pulled-off while screaming, pitch raises as neck is stretched
Blood Sucking Freaks - the brain sucked out of woman's skull ... wait a minute, that doesn't count... sorry
**actually Savini discussed the Maniac head trick - fired both barrels at a blood condom and entrail filled replica of his own head, not a watermelon**

If my memory serves correctly, I was impressed by the suicide head-shot in The Devil's Advocate. I'm not sure if that classifies as a horror movie, though.
